General Forms:

To participate regularly in Solid Rock events it is important for Students and Parents to read, fill out, and sign the below forms for safety and liability reasons. Once these forms are completed they may be turned into Kenny in the Church Office or to Lorri Hymel who is the parent in charge of keeping the records for each student.

Mission Trip Forms:

To participate in our mission trip to Mobile, AL all students and adults must complete and return the forms below. These forms must be returned to Kenny or Mary Knurek as soon as possible. These forms are for Lutheran Disaster Response & Lutheran Children & Family Services of Alabama and are required for all participants working with these organizations. If you have questions on any of these forms please contact Kenny.
